Key Responsibilities:

• Collaborate with the healthcare team to develop and implement care plans.
• Assist in patient assessment and contribute to the development of individualized care plans.
• Educate patients and their families about healthcare conditions, medications, and self-care techniques.
• Document patient care accurately and maintain comprehensive medical records.
• Adhere to infection control protocols and maintain a safe and clean environment.
• Assist with the coordination of patient appointments, referrals, and follow-up care.
• Participate in quality improvement initiatives to enhance patient outcomes

Qualifications:

• Active and unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
• Minimum of 1 year of recent nursing experience in a healthcare setting.
• Knowledge of nursing principles, practices, and procedures.
• Excellent clinical skills and the ability to apply evidence-based practices.
• Strong communication and interpersonal skills to build rapport with patients, families, and healthcare team members.
• Ability to multitask and prioritize responsibilities in a fast-paced environment.
• Flexibility to work different shifts, including nights, weekends, and holidays.
